ABSTRACT

Objective: To explore the feasibility of 3D DSA in guidance of uterine artery catheterization (UAE). Methods: A total of 20 patients, including symptomatic uterine leiomyoma (n=8), postpartum hemorrhage (n=5), scar pregnancy (n=3), adenomyosis (n=1) and advanced cervical cancer (n=3) underwent UAE guided by 3D DSA (3D group, n=10) or 2D DSA (2D group, n=10). The relevant clinical indexes of 2 groups were compared. Results: UAE was successfully performed in all 20 patients (40 uterine arteries). No blood supply from ovarian artery was found. The operation time, catheterization time and fluoroscopy time were both shorter, and the dosage of contrast agent and radiation doses were all lower in 3D group than those in 2D group (all P<0.05). Conclusion: 3D DSA can be used to guid UAE, with shorter operation time and lower radiation dose than traditional 2D DSA.

ABSTRACT

OBJECTIVE@#To explore the effects of simultaneous multi-level surgery intervention on the changes of the oxidative stress and the cognitive function in young and middle-aged patients with moderate-severe OSAHS.@*METHOD@#One hundred and sixteen young and middle-aged patients with moderate-severe OSAHS received simultaneous multi-level surgery. PSG,malondialdehyde (MDA), advanced oxidation protein products (AOPP) and 8-isoprostaneF2α (8-iso-PGF2α) were tested, together with the evaluation of the questionnaires of Montreal cognitive assessment (MoCA), mini-mental state examination (MMSE) and Epworth sleepiness scale (ESS) before and 6 months after surgery.@*RESULT@#(1) The AHI, TS90% and LSaO₂ were all improved significantly 6 months after operation. No severe complications were observed in any patient. (2) The preoperative MoCA and MMSE scores were (23.52 ± 2.64) and (26.54 ± 1.07) respectively. The cognitive impairment were detected in OSAHS patients. MoCA was negatively correlated with AHI, TS90%, MDA, AOPP and 8-iso-PGF2a, while positively correlated with LSaO₂ (P < 0.05, r = -0.233, -0.190, -0.256, -0.247, -0.221 and 0.215, respectively). Cognitive impairment was mainly manifested as disorder in the visual space and execu- tive function, memory and delayed recall (mainly short-term memory). (3) MoCA and MMSE scores were both increased (P < 0.05) after the operation. MoCA showed negative correlations with MDA, AOPP, 8-iso-PGF2α, AHI and TS90%, and positive correlation with LSaO₂ (P < 0.05, r = -0.495, -0.463, -0.397, -0.332, -0.292 and 0.336, respectively).@*CONCLUSION@#Oxidative stress and cognitive dysfunction are closely related in young and middle-aged patients with Moderate-severe OSAHS. Oxidative stress may play an important role in cognitive function of young and middle-aged patients with Moderate-severe OSAHS. Multi-level surgery can improve oxidative stress so that to improve cognitive impairment.

ABSTRACT

Objective To investigate the effects of simultaneous multi-level surgery for moderate to severe obstructive sleep apnea hypopnea syndrome (OSAHS). Methods A retrospective analysis was made on surgical cases of one hundred and thirty seven patients with moderate to severe OSAHS diagnosed by polysomnography (PSG). They were divided into multi-level group (n = 95) and UPPP group (n = 42). The two groups were compared in terms of postoperative complications as well as the related indicators of PSG , calgary sleep apnea quality of life index (SAQLI), epworth sleepiness scale (ESS), snore scales (SS) before operation and after operation. Results Just one patient in the multi-level group had difficulties in respiration and was rescued by timely tracheotomy. The AHI, LSaO2, TS90%, the total score and the scores on the four dimensions of SAQLI, ESS score, SS score in the multi-level group were significantly improved as compared both to the results after operation (P < 0.01) and to the UPPP group (P < 0.05). But only the AHI, LSaO2 and TS90% in the UPPP group were improved (P < 0.05). Conclusions The multi-level surgery is a safe and feasible therapy or moderate to severe OSAHS. The evaluation in subjective and objective ways can be more accurate in comprehensive reflecting the surgical efficacy and effects of OSAHS on patients′ of life quality.

ABSTRACT

BACKGROUND: Systematic family therapy focuses on the interaction, and group counseling contributes to the improving of interpersonal relationship. To introduce the ideas and techniques of systematic family therapy into school situations, which is necessary to the peer relationship improvement of disliked children in the class.OBJECTIVE: To study the feasibility of introducing the ideas and techniques of systematic family therapy into school situations with the aim of improving the peer relationship.DESIGN: Inequality experiment group and control group design.SETTING: Institute of Educational Sciences and Administration, Yunnan Normal University.PARTICIPANTS: The experiment was carried out in Kunming No.l Primary School from September 2000 to April 2001. Two out of three ordinary classes in Grade Six were chosen to take part in the experiment at the school. Both classes were similar in performance, total number and composing. Class 3 of Grade 6 (n=46) was made experiment class randomly, and Class 1 of Grade 6 (n=41) was comparison class. Children of both classes were measured by the Social Distance Scale for Children at the same time, which was described in 5 grades. All children were required to evaluate their classmates by the list. The following 5 grades respectively scored 5, 4, 3, 2 and 1 point: making best friend with him or her willingly, hope to be with him or her in the same class but not the best friend, being with him or her willingly but not many times or too long, he or her may be in our class but have no intercourse, hope him or her not to be in our class. The evaluation result was statistically analyzed, and the higher the mean score of child had, the more popular he was. Five male children who was the farthest from their peers in the experiment class were taken as the experiment group according to the score of Social Distance Scale for Children, who were 12-13 years old with a mean age of 12.7 years. Another 5male children in the other class were selected as the control group by the same way, who were 12-13 years old with a mean age of 12.5 years.METHODS: The idea of systematic family therapy was introduced into the experiment group, including 10 times of group counseling, which were carried out by games, role-playing, group discussion, brain-storming etc.The "everyday work" was assigned to the class as the techniques of systematic family therapy required, especially for children in the experiment group such as "keep secret progress account", and teachers and classmates were required to watch and note their members' progress secretly. While children in the control group and corresponding class were given no intervention measure. Children of both classes were tested by the Social Distance Scale for Children to evaluate the changes in peer relationship during and after the experiment respectively. And children in the experiment group were required to fill in the Group Activity Viewpoint Questionnaire.MAIN OUTCOME MEASURES: Comparison in score of Social Distance Scale for Children before, during and after the intervention, and the investigation on viewpoint about group activity in the experiment group.RESULTS: A total of 10 children in both groups were involved in the analysis of results. There was no significant difference between children of the two groups in social distance before intervention and during it(t=0.232,1.089, P > 0.05). It cotld be Iearnt from the experiment after test that the score in children of the experiment group was remarkably higher than that of the control group (t=3.169, P < 0.05). The effect of group activity on children in the experiment group was remarkably clear and excellent.CONCLUSION: It is effect and feasible to introduce the idea and techniques of systematic family therapy into school situations, aiming at improving the peer relationship of children.
